Pricing

A referral from primary care provider required to ensure continuity of care for your total healthcare needs. Please check our accepted insurance list. If you are referred to us from an entity not participating with insurance and you have active insurance coverage with a carrier we are contracted with we are required to bill them. Therefore your referral needs to follow your insurance coverage rules and guidelines. Please check with your insurance carrier. Authorization for in-network or out of network coverage for initial consult will need to be obtained by the referring PCP prior to scheduling.

Price given does not include outside laboratory fees that may apply to your service. You will be billed separately for laboratory services. We do not have access or pricing for labs.
